When you go to your appointment to apply for a new passport, you will need to bring the following items with you to the post office:

  1. A completed U.S. Passport Application form (DS-11)
  2. Proof of U.S. Citizenship. You will need to present original physical documentation proving your citizenship status (for example, your birth certificate). In addition, you will need to bring a photocopy of the document.
  3. Proof of Identity. You will need to show, for example, a driver's license. In addition, you will need to bring a photocopy of this identification document.
